Now Taking Self-Referrals: Autism & Learning Disability Liaison Team

The Oxfordshire Autism and Learning Disability Liaison Team (ALDLT) is now taking self-referrals for young people diagnosed with Autism. This includes parent carers and young people themselves.
ALDLT is a team of Care Navigators who provide support to ensure children, young people and families get the right support at the right time, and that local systems are responsive to meeting their needs in a holistic and joined up way. They have specialist knowledge and skills in working with people who have a Learning Disability, Autism or both. They are not a clinical team, which means a child or young person's care remains co-ordinated by the CAMHS pathway that they are already working with.
To be eligible for referral, a young person must:
Be aged 0-25 years.
Have a diagnosis of a Learning Disability or Autism or both.
Be on the Dynamic Support Register (DSR).
Be at risk of hospital admission, currently be in hospital and/or at risk of placement breakdown.
